We will coordinate your care during your hospitalization, working with many other members of the hospital team, including your nurses, social workers, therapists, pharmacists and consulting physicians.
We will arrange your hospital discharge or assist in coordinating your discharge if we are a consulting physician.
We will communicate with your family physician. He or she will receive a copy of our admission or consultation note as well as a copy of our discharge documentation. This provides a summary of your hospital care, lists your medical problems, discharge medications and plans for follow-up after discharge from the hospital. This is faxed to your doctor's office when you are discharged from the hospital, ensuring that your family doctor has crucial information for providing your ongoing care.
